Outpatient clinic

An outpatient clinic is designed for treatment of patients who come to the hospital without the intention of staying beyond the visit. The patients in this clinic are usually there for diagnosis and treatment, without the need for a bed or overnight care (Kunders, 2004).

There are as many purposes of the outpatient clinic as there are specialists at the clinic. Hence the outpatient clinic will have a wide range of diagnostic services, treatment services, and minor surgical procedures. Run by consultant physicians and surgeons, the outpatient clinic in a hospital is highly integrated to the inpatient clinic(Carr, 2017). Some of the patients who attend the outpatient department end up as inpatients who are admitted to the hospital ward. Once discharged from the hospital wards, the patients may visit the outpatient clinic for follow-up treatment.

Populations visiting the outpatient clinic are usually patients from the population served by the hospital. So for a community hospital, people from the community will be the ones seeking diagnostic and treatment services in the outpatient department. These people seek the services for a range of diseases, with the consultants offering these or referring the patients to specialists in other locations.

Most outpatient clinics are located on the ground floor for easy access. They usually have a car park nearby. There are also wheelchairs and stretchers at hand. A reception will have a waiting area nearby, with other nearby facilities been toilets, coffee shop/snack bar, gift shop, florist, quiet room and water dispenser. Each physician will have a consulting room. Other facilities in the outpatient clinic include laboratory, X-ray, medical records and pharmacy(Carr, 2017).

Notably, there are outpatient clinics independent of hospitals. SO they are complete healthcare centers that will give a good account of the essential requirements of a healthcare facility.

Public health structure and function that incorporates the three levels of government – national, state and local – is not only important for it to be able to respond to such crisis as pandemics, but also to be able to address other ongoing issues such as public safety, chronic illnesses and health promotion. In the United States, there is no comprehensive national health policy that defines the structure and functions of the federal, state and county governments. Yet the state offers tenders for supply of the medical equipment whilst the hospitals under the jurisdiction of the local governments provide the diagnosis and therapy including home visits to asthma patients (Boyd-Barrett, 2020).

The constitution provides for a federal government that divides power among the legislature, executive, and judiciary; with the state governments adapting similar schemes. With regard to health, the legislature enacts health policy and allocates the required resources. Health departments and agencies within the executive implement the legislation and enforce the policy through appropriate regulations. The judiciary resolves any legal disputes by interpreting the health laws as enacted by the legislature (DeSalvo, 2017). The judiciary arm at the state and federal levels has especially been influential in defining the powers of the government in health matters. Hence the various arms of the government at the various levels provide checks and balances, offering oversight and control of the other arms. For example to determine what quotas of ventilator and ventilator circuits to be allocated to asthmatic children in California versus quotas for Covid-19 requires consensus among the Californian legislature, executive and judiciary (Boyd-Barrett, 2020).

Hence, the federal government is involved in public health in the areas of  “policy making, financing, public health protection, collecting and disseminating information about U.S. health and health care delivery systems, capacity building for population health, and direct management of services” (Institute of Medicine, 2003). Providing futile medical treatment

The ever-emerging new and innovative treatment options continue to offer hope to patients and their families that hitherto seemingly untreatable conditions may be treatable through more aggressive treatment and that there are improved end-of-care options. Knowledge of this by the patient through the ubiquitous information channels (e.g. Internet and mass media) has led to unprecedented patient sophistication, participation and expectation regarding their clinical situations. Because of this, patient’s autonomy, rights and medical ethics have become important issues. This is especially pronounced where the physician views further treatment as futile whereas the patient and their family are still expectant of a therapeutic solution (Willmott, White, Gallois, Parker, Graves, Winch, Callaway, Shepherdand Close, 2016). Such a situation raises clinical and ethical dilemmas.

Suffice to say there is no agreement on what constitutes “futile treatment” as there are often prognostic disagreements among physicians. The threshold for futility varies, with some physicians deeming 20% therapeutic benefit as futile while others consider it as futile when there is 0% benefit. Moreover some physicians want to try new treatments on the patient. Other physicians may fear admitting failure whereas others do not want to “abandon” the patient, having an emotional attachment. Hence there is confusion, controversy and debate as to what constitutes futile treatment(Howard and Pawlik, 2009).

Further, medical treatment futility is determined by the therapeutic goal. The goal could be to heal, to improve the quality of life or for end-of-life care. The patient and physician may not agree on the therapeutic goal, so this requires open communication and close collaboration between the two parties (Rubin and Bonomo, 2016). It is important tobe clear what the treatment aims to achieve so as to determine when the treatment is no longer useful.

Patient and the family hardly agree with the medical professionals about the futility of a treatment, with a tendency for families to want to prolong the treatment as a sign of care or love or as a compensation for their past neglect of their relative. In such situations, open communication from the start of the treatment between the two parties is important so as to set limits and to determine how to judge success or lack thereof. Treatment should be aimed at providing healthcare rather than impressing laypeople; be they the family, insurers or lawyers(Neville, Wiley, Yamamoto, Flitcraft, Anderson, Curtis and Wenger, 2015).Frank discussions and close collaboration between the parties also leads to suitable accommodations. Such accommodation may include prolonging the treatment to abate death of a pregnant woman until birth of a child or even supporting the patient to have good quality of life over Christmas period.

Moreover, if the family persists on pursuing treatment despite its futility, they may be encouraged to seek other opinions. As noted earlier, different physicians do not often agree on the futility of a treatment. Obstinate patient and family may be encouraged to pursue treatment with the physician(s) who feel the treatment is still beneficial.

When treatment is deemed futile, there is no professional obligation or government legislation requiring medical professionals to continue providing treatment(Willmott et al., 2016). ·         Provide high performing narrower networks by using different cost sharing levels that offer right care at the right place at the right cost and ensure better quality.

·         Apply pharmacy solutions by encouraging using generic names instead than the brand name.

·         Use alternative funding such as wellness programs and cost saving rewards.

·         Use right level of care by encouraging right behavior in achieving care and offering plans based on medical information as the basis for plans with built-in incentives to guide members toward following medically proven practices.

·         Employee cost sharing by building additional cost-sharing structures into their health plans, by adding more of the costs in addition to the covered employees.

Why should that strategy work?

In my opinion the most effective strategy is the strategies that provide healthcare costs based on the employee previous health plans and medical needs (consumer driven health plans). This will be more cost-effective as it will provide the needed cost only and ensuring providing the appropriate care to the patient.
