            The prevention of adverse effects and medical errors, associated with healthcare service provision is one important way of improving healthcare quality (Youngberg, 2011). Although healthcare has become more effective, the increased use of technology and numerous healthcare policies have created a complex healthcare system. The modern healthcare services are offered to an increasingly aging population, who possess significant co-morbidities that demand difficult decisions. Moreover, the increasing economic pressures on healthcare organizations have become a major source of workload that compromise quality of care.

            The quality of healthcare can be improved by focusing on improvement of patient safety in healthcare systems. According to (Youngberg, 2011) a safer patient care can be attained through the use of patient safety theories and application of measures in clinical practice. Some of the safety initiatives that can be employed by the healthcare organization include reporting of errors, which is the first step towards learning from errors (Galt & Paschal, 2011, pp. 134), using healthcare teams and training of healthcare providers on patient safety (Salas & Frush, 2013, pp. 3-19) and increased awareness among others.             Although patient safety is the most fundamental aspect healthcare provision, provision of safe care is not enough as this should be provided in time. There is need for healthcare organization to reduce wait times in the provision of healthcare services.             The healthcare reform in the United States has a rich history, where almost every president has tried to initiate some reforms since Second World War. However, the healthcare system in the United States still struggles with perennial problems. President Bush, whose main agenda was to tame the industry’s increasing costs and improve quality of healthcare through information technology, initiated reforms in the industry. However, his efforts bore less fruits since the healthcare costs kept increasing. The concern of increasing healthcare costs were more evident as one of President Obama’s campaign agenda was to increase healthcare coverage, reduce costs of healthcare and improve its quality.

            Although the healthcare sector in the United States is struggling with other underlying problems, for example nurse shortage, discontinuous healthcare coverage, the problem of the ever-increasing costs of healthcare has attracted more attention from researchers and public. The federal funding for healthcare stands at almost 16% of the country’s GDP, making it the highest in the world, compared to other developed countries. In addition, this makes it among the single largest allocation for a single sector in the country. Many researchers and players in the healthcare sector blame the rising costs of healthcare on the expensive medical equipment that add less value, increasing chronic conditions, aging population, increased administrative costs due to the recently introduced healthcare insurance options and the disjointed healthcare industry among other factors.

            The rising healthcare costs are unsustainable and measures need to be taken to control the problem. Therefore, this report was drafted in order to look into the underlying problem of increasing healthcare costs in the US and compare possible solutions as well as give recommendations that would help to solve the problem.

            In order for one to develop a good understanding of the healthcare system in the US, its key milestones and challenges, it is important to understand its history. The hospitals in the Western world have evolved from charitable home cares into centers of scientific and academic excellence all largely to the influence of cultural and social developments.

            Hospitals in the US can be traced back to the nursing homes that offered patient care to poor patients and they were run by charities (Harrington, 2011). Most of the patients included those who suffered from ailments such as leprosy and retirees. A patient who wanted to be admitted had to show his/her moral worth sufficiently or a benefactor.

            During the 19th century and most of the 20th century, many of the patients in the US received their healthcare needs such as surgical procedures, treatment of diseases and birth at home (Wall, 1998). A large number of the Americans lived in their rural homes and they did not find an opportunity to visit healthcare practitioners who were mostly situated in the urban centres. The charitable traditions in the healthcare participation were deeply rooted. However, in the end, municipalities were developed, which stripped the community of its services of providing care for those patients who suffered from chronic illnesses such as lung diseases as well as from providing care to patients with disabilities.

            In 1736, a six-bed room charity nursing home was founded in New York, which would be turned into Bellevue Hospital. In 1829, one of the country’s oldest hospitals, Regional Medical Center, in Memphis, Tennessee was founded (Galbraith, 2006). The same year witnessed the formation of other institutions and powerful municipalities. Although many will regard the homes for the elderly in the 1960s that developed due to the American civil war as the origin of American’s hospitals, there were hospitals at the time, which were staffed with nurses, doctors and specialized departments. However, the American civil war, industrial revolution, advancement in medicine and social development led to the development of hospitals in the country. In fact, by 1965, over 30% of smaller hospitals and almost 90% of bigger hospitals had intensive care units (Wall, 1998).  In the early and middle 19th century, a period termed as “therapeutic pessimism”, doctors could not treat diseases adequately.
