Case Study 1
The patient, Christopher, is a 45-year-old Mauritian male of African origin who entered treatment voluntarily at Centre Idrice Goomany which is an urban residential substance use treatment centre in Plaine-Verte.
Presenting complaints
Christopher reports that he relapsed to substance use 8 months ago, and his primary drug of choice is crack/cocaine accompanied by frequent alcohol use. He reports that he had success in treatment for the first time after he was released from prison 4 years ago, and he would like to try and get back on track, as he has hit a low point in the past few months. At the time of treatment entry, Christopher reports no stable living arrangement and had recently been splitting time at the homes of his friends, ex-girlfriend, and uncle. He has four children between the ages of 10 and 27, with three separate women, and has intermittent contact with each of them. He reports that his eldest son, was murdered 5 months ago in a drug-related incident. In his current environment, he reports spending most of his time with old friends, with whom he re-established a relationship with after he was laid off from his job 9 months ago.
History
Christopher was raised by his mother and never met his father. He reports an extensive family
history of substance use, including heroin and crack/cocaine use by his mother, who passed away 10 years ago. Christopher dropped out of high school after Grade 9 (Form III). Between the ages of 18 and 40, he was arrested and spent time in prison multiple times for charges related to theft and possession of marijuana and cocaine. He was released from his last prison term at the age of 41. He has worked primarily as an assistant electrician over the course of his life, but he refused to provide additional details regarding his employment history. His most stable employment history has been during the past 4 years after completing a court-mandated treatment at Centre de Solidarité in Solitude, a residential substance use treatment facility. He reports that he was committed to “turning my life around” at this time, and he successfully remained abstinent until approximately 8 months ago.
Clinical interview
During the interview, Christopher displayed psychomotor retardation, clear thought processes, and no obvious perceptual abnormalities. His speech volume and tone were within normal limits, yet his speech rate was somewhat slower than normal. He reported past crack/cocaine and alcohol dependence beginning at age 16. He reported feelings of depressed mood most of the day, nearly every day, markedly diminished interest in almost all activities, feelings of worthlessness and excessive guilt, and a diminished ability to think or concentrate.
Following the loss of his job, he reported that he had a lot of free time and got bored easily. He felt worthless that he couldn’t find a job and often ruminated over the guilt he felt about his choices in life and his inability to provide for his family. He contacted his old friends, which was soon followed by cocaine and alcohol use. He also reported feeling lonely, guilty, and worthless following the loss of his son, and drug use helped him “get rid of” these feelings, although they would always resurface when he was sober, leading to a cycle of negative reinforcement. Finally, he reported intensifying feelings of sadness and shame that he had used crack/cocaine and alcohol again given how much progress he had made following his release from prison. He felt that he had let his family down, as he was just starting to re-establish relationships with his children before his relapse.

Hoover (2016) points out that electronic health records (EHRs) have allowed the integration of technological features such as bar code scanning that allows physicians to provide correct medications to patients.             One of the key in practicing as a psychologist is to conduct a holistic psychosocial assessment, in order to evaluate effectively the problem of a client (Perkinson, 2012). To achieve this goal, proper interviewing techniques and appropriate assessment tools are employed. An effective interview often employs effective interviewing strategies in order to get relevant information regarding the client problem.

The Interview Strategy that Was Modeled Effectively

Based on the observation of the addictions professional in the scenario presented in the media file, the interview strategy that was best modeled was the evaluation of the patient childhood experiences. The interview began by asking the patient where he was born and his childhood relationships with his parents. In doing so, the interview was able to get good information regarding the genesis of the client problem.

Moreover, the interviewer proceeded to ask questions related to perceptions of the client towards his father, mother and brother. The information provided by the client regarding the questions provided effective Biopsychosocial assessment of the client. The way the interview questions unfolded depicts a lucid assessment of the client. This particular interview strategy was found to be effectively modeled since it provided a clear understanding of patient past childhood growth and its probable impact on his later adulthood life.

The Interview Strategies that Would Be Avoided           Based on the observation of the media file, the interview strategies that would be avoided is asking the reason that brought the client in and delving directly into his problem without effective introduction. According to (Greenfield, & Hennessy, 2015) an effective interview assessment must start with an appropriate introduction. A good introduction enables the psychologist to win the confidence and trust of the patient. Introduction

            The healthcare and public healthcare system in the US has undergone major reforms. It is prudent that the nursing workforce is able to meet the changing and increasing healthcare reforms. There have been many reports that have focused their attentions on “what should be” rather than “what is”. All these reports have studied the capacity of the workforce in the healthcare to meet future demands in the sector. The healthcare system in the US has evolved greatly and there is need to educate nurses on the need for continuum in order to meet these changes.

Evolving Practice of Nursing and Patient Care Delivery

            A thorough examination of the capacity of RN to meet the ever-changing healthcare system in the US is justified by many reasons. The administration of Obama, for instance, introduced many changes in the healthcare system, (Nap.edu, 2014). These changes include the legislation reform in the healthcare signed by the president in 2010 and the American Recovery and Reinvestment Act of 2009. All these changes are designed to change the way the healthcare is financed and how the services are delivered. The RN nurses should be acquainted with these changes in order to ensure that they are most competent and able to meet future changes and challenges.

            Most of the organizations under the new programs introduced by the Obama administration, for example, the Accountable Care Organizations (ACOs) and the Primary Care Medical Homes rely entirely on the services of the RNs. The RN are tasked with the responsibilities of coordination and transition care. This points the importance of RNs in the success of the reforms in the healthcare both currently and in the future. Furthermore, the investments in interoperable health information technology (HIT) form critical core area in the implementation of major reforms in the healthcare system. In addition, interoperable health information technology (HIT) has an effect of influencing the growth of the information exchange in the community, potential of changing the skill mix, distribution and the scope of the RNs in many ways.

            Therefore, the questions to be answered are whether the demand for the services provided by RNs will change as suggested by the healthcare reforms. Furthermore, has nursing workforce been positioned effectively? Moreover, what does the future ask of the nursing workforce? The implementation of these reforms and related policy frameworks has a significant impact on the roles of the RNs and their requirements.

Advancing the Care Management Models

            Care Management are a collection of strategies meant primarily for individuals under critical illness and whose services require intervention of a number of healthcare providers with long-term and continuous management. The most dominant care management in the past 15 years in the country has been disease management and this has been responsible for huge spending in the healthcare. The program heavily relies on the good coordination and interdisciplinary clinical management for it to be a success.

            Medicare Prescription Drug Improvement Modernization Act of 2003 made a number of provisions in population based care programs to assess whether cost saving could be achieved in its pilot programs.             The demand for advanced practice nurses and RNs will keep growing since there will be increased accessibility, coverage and service setting as well as services. However, culture is dynamic and the competence of any RNs is based in his/her cultural competence. The diversity of the culture in the US colleges has created even a rich source of students with diverse backgrounds.
