EBP is crucial to nursing practice because it incorporates the best evidence from current literature, along with the expertise of the practicing nurse. The concern for quality care that flows from EBP generates a desired outcome. Without these factors, a nurse cannot be an effective leader. It is important to lead not only from this position but from knowledge and expertise. To gain the knowledge, you require a good understanding of how to search for scholarly resources, as well as identify which databases and websites are credible for the purposes of implementing evidence-based changes in practice.

Scenario

You are supervising three nurses working on the medical-surgical floor of a local teaching hospital. This hospital is nationally recognized as a leader in education and has a computer lab with an online library where staff has access to medical research databases (that is, CINAHL, PubMed, Medline, and Cochrane library) and online sources of all hospital policies, procedures, and guidelines, and computers at nurse workstations that also have access to these resources. (For this scenario, use the Capella University Library to simulate the hospital’s online library.) You have given the nurses their patient assignments and you have all participated in shift report. A new nurse who just completed orientation and training a week ago approaches you and tells you that one of the assigned patients has a diagnosis he or she is very unfamiliar with. Knowing that patient-centered care based on best practices is imperative to positive patient outcomes, you want to assist this nurse to find research that can be utilized to provide the best care for this patient. Describe how you would communicate with this nurse to encourage him or her to research the diagnosis. Assume you will assist in the quest to locate evidence, then describe where you would go within the facility and what resources you would look for. These resources may include websites, journals, facility policies or guidelines, or any other sources of online information.

You may choose the diagnosis for the patient in this scenario or health care issue/diagnosis of your choice. Or you may choose something you would find interesting to research or that applies to a clinical problem/diagnosis you would be interested in addressing. Create a list of at least five sources that could be used to find evidence, with the best source listed first, and explain why the sources you chose are best to find evidence for the diagnosis you chose and the clinical scenario. You are only evaluating the sources of evidence (database, website, policy database or website, journal article, et cetera). You are not actually completing a search and selecting evidence. Consider the following examples: a nursing journal in CINAHL may not be the best source of evidence for information on how to administer medications through a central-venous catheter, whereas a hospital policy database found on a website may not be the best source of information on caring for a patient with a rare chromosomal abnormality.

Instructions

The purpose of this assessment is to understand where to find evidence that can be applied to clinical scenarios and to learn effective communication and collaboration with clinical staff during the process of evidence location. As a baccalaureate-prepared nurse, you will not only use research for self-improvement in your clinical role, but you will also serve as a mentor to supervised nursing staff. Therefore, you will need to be able to communicate and collaborate effectively to guide them toward resources to find research, as well as support them through the initial evidence location process. In doing so, nurses can gain access to evidence that can be analyzed and utilized to stay current on best practices. This allows them to provide safe, patient-centered care and improve patient outcomes.

For this assessment:

  • Describe your role as a baccalaureate-prepared nurse supervising clinical staff nurses with regard to communication and collaboration in locating evidence for application to a nursing practice scenario.
  • Compile a list of five online databases or other online sources (that is, websites, journals, facility policies or guidelines, et cetera) that can be used to research evidence to apply to this scenario or clinical practice issue/diagnosis and describe to which of these you would direct a nurse colleague to search for evidence.
  • Describe where you might go in the work place to complete this research and how you would access the desired, relevant research within research databases or other online sources.

Be sure to address the following in this assessment, which correspond to the grading criteria in the scoring guide. Please study the scoring guide carefully so that you will know what is needed for a distinguished score.

  • Describe communication strategies to encourage nurses to research the diagnosis/practice issue, as well as strategies to collaborate with the nurses to access resources.
  • Describe the best places to complete research and what types of resources you would want to access to find pertinent information for the diagnosis/health care issue within the context of a specific health care setting.

Note: While you are not selecting and evaluating specific evidence to help with the clinical diagnosis/practice issue, you should still be citing the literature and best practices to support your description of your communication and collaboration approach. Additionally, it is appropriate to cite best practices related to EBP and evaluating databases to support your explanation as to why you selected the five sources of online information that you did.

Your assessment should meet the following requirements:

  • Length of submission: 2-4 pages (not including the title page or the reference page) description of communication, collaboration, and evidence location process, including a list of databases or other sources with description of why they are appropriate for clinical scenario diagnosis/health care issue (that is, something that would be useable in professional practice for other nurses). Be sure to include an APA-formatted reference page at the end of your submission.
  • Number of references: Cite a minimum of three sources of scholarly or professional evidence that supports your findings and considerations. Resources should be no more than five years old.

    Task Details For this assessment you will apply and contextualise advanced knowledge and understanding of Australian healthcare law and/or ethics that impacts on your workplace, through contributing to E-tivities and an annotated bibliography.
    PART A – Essay – E-tivities:
    Format: You will submit evidence of participation in the E-tivities by copying and pasting your chosen contributions from the Study Desk E-tivity Forums onto a Word document file for submission. Please use a heading for each E-tivity indicating the week it came from.
    Guidelines for E-tivities
    • Each week, an E-tivity will be assigned as part of your weekly activities. Read what the activity is asking you to do and post your response on that week’s Etivity Forum.

Nursing plays a role in changing health policy on multiple levels: conduct quality improvement projects on the job, attend local meetings to share their expertise and advice and write to their political representatives to advise them on critical health-related issues. Registered Nurses and Advanced Practice Registered Nurses are in a great position to advocate for policy development because they spend so much time with patients to know the specific needs of the communities they serve. (Regis College, n.d.).

 Challenges

Nurses often must move out of their comfort zone to advocate for health policy changes, and this process can be very time-consuming. Some have no interest in politics and policy-making and lack educational preparedness. Some have found a lack of resources and mentorship from exemplary nurse leaders. 

           Joining nursing organizations is essential for nurses to enhance individual advocacy efforts. These groups provide resources and strategize more effectively to bring nursing perspectives to health policy decision-makers. Nursing organizations can monitor public policy, offer ways for their members to learn about health policy, and serve as a resource for reliable information related to policy issues and policymakers. (Abood, n.d.).

 Strategies 

As nurses, these opportunities to participate in policy-making can be communicated in the employment setting through word of mouth directly to coworkers or nursing-led education classes about the subject. Also, social media is a great way to reach, educate, advocate, and communicate with a specific target audience on a larger scale in this day and age. (Anders, 2021).

Registered nurses (RNs) and advanced practice registered nurses (APRNs) play a vital role in healthcare delivery. As RNs and APRNs, we must become actively involved and continue to be the voice and advocate for our patients and community. We can become involved by meeting with local legislators and community leaders to address the problems we are facing with health care. Also, joining an organization such as American Nurses Association is a way for nurses to become actively involved in the policy-making process. In addition to lobbying on issues that directly affect the nursing profession, ANA takes bold stances on issues important to its members. ANA has taken firm positions on a range of issues including Medicare reform, the importance of safe nurse staffing, workplace violence, whistleblower protections for health care workers, and adequate reimbursement for health care services (ANA, n.d.). Social media is a key platform for nurses to be advocates. For example, Facebook has a variety of distinct groups designated for RNs and APRNs. It is a resourceful tool to utilize to network and get information to the public. Another strategy in increasing participation and bringing awareness to politics and policymaking is providing nurses with efficient information regarding health policies. For example, I did not have a lot of knowledge about healthcare policy making until taking this course. Surprisingly, this information is not provided or taught in undergraduate courses. If the information is introduced at an early stage in nurses’ careers, then they will be more actively engaged.

Nurses are not always granted the opportunity to voice their concerns. The International Nursing Review article identifies a lack of support, resources, and time for nurses to do so in their workplaces, and too often nurses lack confidence and skills in policymaking and do not understand the differences or connections between policy and politics (Health Stream, 2021). However, nurses should not allow negativity or lack of support from legislators to discourage them from becoming involved. Instead, nurses should fight back and have adequate evidence-based practice to support the topic of concern. Nursing organizations allow nurses to band together and support one another in policy implementation. Nurses can create healthcare change and should become involved in the policy process and learn about upcoming issues. As a component of professional nursing, active participation in the policy process is essential in the formulation of policies designed to provide quality health care at sustainable costs to all individuals (Milstead & Short, 2019).

Lewis Securities Inc. has decided to acquire a new market data and quotation system for its Richmond home office. The system receives current market prices and other information from several online data services and then either displays the information on a screen or stores it for later retrieval by the firm’s brokers. The system also permits customers to call up current quotes on terminals in the lobby.

The equipment costs $1,000,000 and, if it were purchased, Lewis could obtain a term loan for the full purchase price at a 10% interest rate. Although the equipment has a 6-year useful life, it is classified as a special-purpose computer and therefore falls into the MACRS 3-year class. If the system were purchased, a 4-year maintenance contract could be obtained at a cost of $20,000 per year, payable at the beginning of each year. The equipment would be sold after 4 years, and the best estimate of its residual value is $200,000. However, because real-time display system technology is changing rapidly, the actual residual value is uncertain.

As an alternative to the borrow-and-buy plan, the equipment manufacturer informed Lewis that Consolidated Leasing would be willing to write a 4-year guideline lease on the equipment, including maintenance, for payments of $260,000 at the beginning of each year. Lewis’s marginal federal-plus-state tax rate is 25%. You have been asked to analyze the lease-versus-purchase decision and, in the process, to answer the following questions.
